I have a small top and wide hips. I usually wear a 4R in dresses. The 4R fit a bit better but was super short so I wore the 6R. I wish I had ordered as my second size the 4L. There is some stretch to the dress so it looked fine. The dress is forgiving on fit and can stretch or shrink depending on your body shape. I wore my normal undergarments so there is no need for a special purchase. Also I wore a normal heel. Overall the dress was pretty and easy to wear.
